Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br />any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. All new addenda supersede the previous ones. <br />3. Provide written notification to ustCa�siaov.ora to arrange the following repairs and testing (a 48- <br />hour notice is required). Please be aware that inspections for emergency work must be <br />scheduled within five days of permit approval. <br />a. Leak detector test: Leak detector must be tested to confirm that the device can detect a <br />leak of 3 gallons per hour at 10 psi. <br />4. Any individual performing the work of a service technician must meet the following <br />qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible valid, <br />current contractor's licenses issued by the Contractors State License Board (may be <br />issued to the installation company), or a Tank Tester's license issued to the individual <br />by the State Water Board <br />b. Possess or work under the direct and personal supervision of an individual physically <br />present at the work site who possesses a valid, current California UST Service <br />Technician certificate issued by the International Code Council (ICC) <br />c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced <br />5. The service company must provide the following documentation to the tank owner/operator for <br />an inspector to review during the annual UST inspection: <br />a. Description of parts) installed (parts list, cut sheets, etc.) <br />b. Name & UST ICC # of the service technician performing the work (ICC, manufacturer <br />certification proof, etc.) <br />1868 E.
