The tables from the respective reports summarizing <br />the investigation included with your letter do indicate that some soil contamination is <br />present. <br />Enclosed please find the "Master File Record" information form, a "Billing and Compliance <br />Acknowledgement" form and the fee schedule. Submit the completed forms with the $234 <br />"Environmental Assessment" review fee for each site and PHS/EHD will review and <br />comment on the reports. Any staffing time used beyond the three hour minimum will be <br />billed at $78 per hour. <br />In addition, soil sample results of soil collected beneath the waste oil tank at 4315 Waterloo <br />Road removed in December 1986 have never been submitted to PHS/EHD.
