West Lane Acct I D#: 0017786 <br />Stockton, CA 95205 <br />RO#: 001793 <br />In a letter dated, April 6, 2006, the San Joaquin County Environmental Health Department (EHD) <br />notified you that if the chemical odors reported to be emanating from the contents of the septic tank <br />located at the above referenced facility were from disposal of solvents, toxics or hazardous wastes, an <br />environmental assessment would be required. On April 13, 2006, EHD received the laboratory <br />chemical analyses results from the sample taken of the contents of your septic tank. The results show <br />that there was a hazardous waste disposal into the septic system, which is prohibited by San Joaquin <br />County Ordinance Code Section 9-1110.7. <br />EHD has implemented a Local Agency Groundwater Protection/Underground Injection Control (UIC) <br />Program and has placed your site on our list of UIC sites in San Joaquin County. In accordance with <br />guidelines prepared by the United States Environmental Protection Agency (USEPA) it is required that <br />the contents of the septic tank be removed, properly disposed and that an investigation be conducted <br />to determine if the soil and/or groundwater beneath the septic system has been contaminated by the <br />illegal disposal of the hazardous waste. The investigative reports must be signed by a California <br />certified professional geologist or engineer and the work performed under their charge. <br />The following items are to be submitted to EHD in order to begin this work: <br />A completed, signed Master File Record (enclosed) and a check made payable to EHD for <br />$279. (This will be used to create an EHD account and pay for the first 3 hours of oversight <br />time. You will be invoiced monthly for EHD oversight time. Our current hourly rate is $93.) <br />A detailed site map of this parcel showing the underground pipes, tanks, sewer disposal <br />system (clarifiers, tanks, etc), and the storm drainage system servicing this site (pits, dry <br />wells, municipal hookups, etc). <br />A work plan to collect soil samples in the first two feet of native soil under the septic <br />tank/clarifier and every 20 linear feet of any subterranean pipes leading into or out of the tank. <br />Soil samples are also required to be collected from under any distribution boxes (D-box) and <br />from the bottom of any seepage pits associated with these systems. <br />Please contact Michael Infurna at (209) 468-3454 if you have any questions. <br />Donna Heran, REHS, Director <br />enclosure: to addressee only <br />C: <br /> AGE — Chris Miller, 837 Shaw Rd,Stockton, CA 95205 <br />C: USEPA Reg 9— E.
