• <br />ENVIR <br />SUBJECT PROPERTY LOCATIONS: <br /> <br />Page 4 <br />April 19, 2012 <br />Project Numbers 2012-04-010 <br /> <br />Three boreholes located at the ends and the center beneath the former underground storage tank <br />location. (B-1 to B-3). <br />A total of three soil boreholes are directed to be drilled. Approximate locations are depicted on Plate 3. <br />Boreholes will be advanced by hand auger until 8 feet or refusal or until ground water is <br />encountered, which ever is less. Sample collection depths will be determined by screening the cuttings <br />with a photoionization detector (PD). <br />Access Agreement <br />Enviro Assess is performing the work for the property owner. A signed access agreement will not be <br />needed before start of work. <br />3. HEALTH AND SAFETY PLAN <br />We anticipate that the health hazards that will be encountered at the Site will be minimal. Based on our <br />understanding of the past disposal practices at the Site, we do not expect to encounter any liquid or solid <br />waste materials nor any liquid or solid hazardous waste materials at the proposed drilling locations. <br />However, if waste materials are encountered, we anticipate that the primary mode of contaminant <br />exposure will be through the skin as a result of handling equipment, soil, and water. Therefore, we have <br />prepared the following Health and Safety Plan: <br />Level D Protection. We propose that all field activities be conducted consistent with <br />Level D protection requirements as defined by the California Occupational Safety and Health <br />Administration (OSHA). The minimal required Level D protection equipment includes hard hat, safety <br />glasses, steel-toed boots, earplugs, disposable rubber gloves, and appropriate work clothing. <br />Emergency Treatment. In the event of an emergency, a member of the team will dial 911 <br />and follow the instructions of the operator. In the event that a non-emergency injury occurs to any or all <br />of the team, they will be transported to the nearest hospital that provides emergency room services (San <br />Joaquin General Hospital, 500 West Hospital Road, French Camp, CA 95231, (209) 468-6000.) <br />4. SOIL SAMPLING PROCEDURES <br />Drilling Details <br />Drilling Method <br />The boring(s) will be advanced with a hand auger. The soil cuttings brought to the surface will be <br />examined and placed on plastic, or in a 5 gallon bucket.
