The missing manifests are: <br /> -012773282FLE <br /> -015004646FLE <br /> -015004645FLE <br /> REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in accordance <br /> with section 66262.23(a)for three years or until the generator receives a signed copy from the designated facility <br /> which received the waste. This signed copy shall be retained as a record for at least three years from the date the <br /> waste was accepted by the initial transporter. The hazardous waste generator must submit a copy of the manifest <br /> to the California Department of Toxic Substances Control (DTSC)within 60 days if the destination facility signed <br /> manifest copy is not received from the transporter or designated facility. <br /> CORRECTIVE ACTION: Locate the the destination facility signed manifests listed above and maintain in the facility <br /> file for three years from the date of shipping. Send a copy to the San Joaquin County Environmental Health <br /> Department(EHD). <br /> This is a repeat violation, Class II. <br /> 303 CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> OBSERVATION: The eye wash bottle was expired in January of 2016. <br /> REGULATION GUIDANCE:All facility communications or alarm systems,fire protection equipment, spill control <br /> equipment, and decontamination equipment,where required, shall be tested and maintained as necessary to assure <br /> its proper operation in time of emergency. <br /> CORRECTIVE ACTION: Bring the eye wash bottle into proper working order by repair, replacement or testing. <br /> Provide a corrective action statement to the San Joaquin County Environmental Health Department within 30 days. <br /> This is a Class II violation. <br /> FA0002890 PR0518567 SCO01 05/13/2021 <br /> EHD 22-01 Rev.9/16/2020 Page 4 of 6 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
