4. Corytinuous monitoring devices <br /> Continuous monitoring devices shad be installed and used to monitor temperature; injec- <br /> tion pressure, flow rate, and volume; and the annular pressure and fluid volume between <br /> the tubing and the long string casing above the packer. <br /> 5. Automatic Alarms, Shut-Off Devices and Operator Requirements <br /> The permittee shall continuously operate and maintain an automatic warning system which <br /> will immediately notify the operators of a malfunction in the event that the injection pres- <br /> sure at the wellhead exceeds the maximum injection pressure authorized by this permit. <br /> This system shall be in operation at the time this permit becomes final. <br /> The permittee will extend the above described system to provide for automatic shut-down <br /> of the well in case of overpressure. This automatic shut-down system will be in operation <br /> no later than 90 days after this permit becomes final. <br /> A trained operator will be on duty during all hours when injection is taking place. <br /> D. REPORTING REQUIREMENTS <br /> 1. Quarterly reports <br /> The perrnittee shall submit short and accurate quarterly reports to the Director containing <br /> the following information: <br /> (a) Results of the injection fluid analyses specified in permit condition Part I1, Section <br /> C, Item 2. <br /> (b) Daily and monthly average, as well as daily and monthly maximum and minimum <br /> values for the continuously monitored parameters specified in Part II, Section C, <br /> Item 3. <br /> (c) Equipment calibration. <br /> (d) A narrative description of all non-compliance which occurred during the reporting <br /> period. <br /> 2. Reports on Well Tests and Workovers <br /> In the first quarterly report after the activity the permittee shall report to the Director the <br /> results of the following: <br /> (a) mechanical integrity tests; <br /> (b) other tests required by this permit; <br /> (c) any well workovers; and <br /> (d) the completion of any conditions included in Part III of this permit. <br /> 15 <br />
