1002566SV,doc <br /> May 26,2010 <br /> Page 2 of 3 <br /> with the approval of the State Representative. <br /> Failure on the Permittee's part to comply with any provision will be cause for revocation <br /> of this permit. <br /> IMMEDIATELY FOLLOWING COMPLETION OF WORK PERMITTED HEREIN, PERMITTEE SHALL FILL <br /> OUT AND MAIL NOTICE OF COMPLETION PROVIDED BY GRANTOR. <br /> 1 Permittee's attention is directed to Section 6, "CONTROL OF MATERIALS", of the State <br /> Standard Specifications Reference to Engineer in the State Standard Specifications <br /> shall include State Representative. <br /> Permittee is directed to Standard Specification section 7-1.11 Preservation of Property, <br /> and Business and Professions Code, Section 8771. Permittee shall physically inspect <br /> the work site and locate survey monuments prior to work commencement. Monuments <br /> shall be referenced or reset in accordance with Business and Professions Code. <br /> A pre job conference with the Permittee, Contractor, and State Representative may be <br /> required prior to start of work. Permittee shall contact the State Representative and <br /> arrange the meeting. This meeting may be waived at the discretion of the State <br /> Representative. <br /> Notwithstanding General Provision #4, your contractor is required to apply for and <br /> obtain an encroachment permit prior to start of work. <br /> i <br /> Permittee shall, prior to commencement of any work, provide the State Representative <br /> with the name and phone number of the person in responsible charge of the work to <br /> be performed under this permit. <br /> Perm itteelcontractor shall work with the State representative to request the necessary <br /> lane closure needed. Request shall be the week prior to the actual work. The State <br /> representative shall submit closure through the Lane Closure System (LCS) for Traffic <br /> Management Center (TMC) approval by Wednesday afternoon of the week prior. <br /> All Lane closures shall be called in by either the contractor to the TMC when the closure <br /> begins (10-97), ends (10-98), or is cancelled (10-22). The TMC can be reached (24-7) <br /> at (209) 948-7556 or 7551. <br /> Use proper traffic control devices throughout the duration of the project as per Caltrans <br /> Standard Specifications. <br /> All night-time work requires COZEEP and shall be coordinated with the permittee or <br /> contractor. The State representative shall advise permittee that COZEEP must be <br /> present since night-time work is only allowed per lane chart. <br /> Before commencing work, permittee must obtain written concurrence from the State <br /> Highway Resident Engineer on the scheduling of the proposed work. <br /> 2 <br />
