The following elements are missing or inaccurate on the site map: <br /> 1.Access and exit points are missing. <br /> 2. Storm drains are missing on the site map. Storm drains were observed outside of the facility building at the time <br /> of inspection. <br /> 3. The two emergency pump shut-offs are not labeled with the chemical that would be shutoff. <br /> 4. The spill response supplies on the site map was not found at the time of inspection. <br /> 5. The location of the carbon dioxide on the site map is inaccurate.The carbon dioxide on-site was observed <br /> outside and on the east side of the facility building. <br /> 6. Loading areas and internal roads are missing or not indicated as"NA"on the site map. <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> OBSERVATION: Deepak Vig,the facility manager,was not familiar with the procedures for the mitigation of a fuel <br /> release.According to Deepak all fuel spills at the facility are cleaned up by a clean-up company. However, <br /> according to Gagan Bahad, the facility franchisee, small fuel spills are cleaned up with absorbent by the facility <br /> employees.A physical copy of the employee training records with dates of training,training discussion points, and <br /> employee names was not found at the time of inspection. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training.All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material, including, but not limited to,familiarity with the following <br /> plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> This is a Class II violation. <br /> FA0025876 PR0546298 SCO01 05/06/2021 <br /> EHD 19-01 Rev.9/16/2020 Page 2 of 3 Hazardous Materials Program OIR <br /> 1868 E.
