S A N J D AR I N Environmental Health Department <br /> lj ;,�J1y <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: FDteEMILS LIQUOR&SPORTS SHOP* 1405 CALIFORNIA ST, ESCALON 17, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION:A large tank of propane on the north side of the store and a cage of 15lbs propane containers <br /> were found on site that have not been reported to CERS. <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> OBSERVATION: Hazardous Materials Inventory has not been annually certified/submitted as a part of the business <br /> plan. The Hazardous Materials Inventory was not submitted for 2019. <br /> REGULATION GUIDANCE: Per H&SC 25508.2, the business owner/operator shall annually review and certify that <br /> the business plan is complete, accurate, up-to-date and in compliance with Section 11022 of Title 42 of the United <br /> States Code.An annual electronic submittal satisfies these requirements.The annual business plan shall be <br /> submitted/certified by January 15 of each calendar year and may be submitted beginning November 1 of the <br /> previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, review and submit/certify the <br /> current information. <br /> This is a repeat violation, Class Il. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. <br /> -Propane was not listed (at entrance and south of building next to side door) <br /> - Internal roads were not listed <br /> -Fuel loading areas were not listed <br /> -Absorbent/spill kit was not listed on the map <br /> -A fire extinguisher was listed to be found on the right of the store entrance but was not found on site <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> FA0000306 PR0520269 SCO01 05/17/2021 <br /> EHD 19-01 Rev.9/16/2020 Page 2 of 3 Hazardous Materials Program OIR <br /> 1868 E.
