COMPLAINANT HAS WITNESSED <br /> THEM CRAWLING ON THE COUNTER ON NUMEROUS OCCASIONS. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Went to facility and spoke with person in charge. <br /> They were not aware of the complaint but they were aware of the problem and currently have pest control in place. <br /> Next pest control service is scheduled for 8/30/21. <br /> Observed 1 live juvenile cockroach on flooring next to ice machine, 1 live adult cockroach on floor under shelves next to <br /> office, and 1 live adult cockroach on the floor in the in office under the desk. <br /> Observed 1 dead cockroach on floor under dishwasher drainboard and several dead cockroaches on floor under spray down <br /> sink for dishwasher. <br /> Observed 1 dead cockroach in file cabinet in office and also observed "speckling"on paperwork. <br /> Clean and sanitize all surfaces throughout the facility. <br /> Increase pest control service as needed. <br /> Re-inspection to occur on or after 9/15/21. <br /> Operator to provide proof of pest control. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.If a reinspection is required,fees will be assessed at the current hourly rate. <br /> Received by: Name and Title: <br /> EH Specialist: STEPHANIE RAMIREZ Phone: <br /> FA0000465 C00054171 SC544 08/25/2021 <br /> EHD 16-23 Rev.06/30/15 Page 1 of 1 Food Program Complaint Inspection Report <br /> 1868 E.
