The financial responsibility <br /> documents on site were dated 12/10/2019 with insurance coverage period of 12/18/2015 to 12/18/2020.The most <br /> current financial documents in CERS are dated 12/21/2020 with an insurance period coverage of 12/18/2020 to <br /> 12/18/2021. <br /> REGULATION GUIDANCE:A copy of current financial responsibility documents are required to be maintained on <br /> site. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the facility's financial responsibility documents and maintain a <br /> copy on site. <br /> This is a minor violation. <br /> 118 CCR 2637.2(d&e), 2665(a&b) Failure to submit the"Overfill Prevention Equipment Inspection Report Form"within <br /> 30 days. <br /> OBSERVATION: Overfill Prevention Equipment testing was performed on 6/14/2021 and a test report was <br /> submitted on 8/23/2021, 2 months late. <br /> REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. <br /> CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are <br /> performed in a timely manner. <br /> This is a minor violation. <br /> 315 HSC 25290.1(e)Failure to maintain interstitial space to detect leaks before release to environment(post-Jul 2004). <br /> OBSERVATION: The S5-91 product high float and S6-91 STP-Fill high liquid sensor failed when tested. <br /> REGULATION GUIDANCE: The interstitial space of the UST shall be maintained, as designed, under constant <br /> vacuum or pressure such that a breach in the primary or secondary containment is detected before the hazardous <br /> substance is released into the environment. <br /> CORRECTIVE ACTION: The sensors were replaced and retested during the inspection. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> FA0007461 PR0506497 SCO01 09/16/2021 <br /> EHD 23-01 Rev.9/16/2020 Page 4 of 5 Underground Storage Tank Program OIR <br /> 1868 E.
