S A N J 0 A Q I I I Irl Environmental Health Department <br /> C C U N T AMENDED <br /> 10/19/2021 <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> 7-ELEVEN INC#14113 3040 W BENJAMIN HOLT DR, STOCKTON September 16, 2021 <br /> Other Violations <br /> 2010 See below Unlisted General violation ❑V ❑R ❑COS <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 118 CCR 2637.2(d&e), 2665(a&b) Failure to submit the"Overfill Prevention Equipment Inspection Report Form"within <br /> 30 days. <br /> OBSERVATION: Overfill Prevention Equipment testing was performed on 6/14/2021 and a test report was <br /> submitted on 8/23/2021, 2 months late. <br /> REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. <br /> CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are <br /> performed in a timely manner. <br /> This is a minor violation. <br /> 315 HSC 25290.1(e)Failure to maintain interstitial space to detect leaks before release to environment(post-Jul 2004). <br /> OBSERVATION: The S5-91 product high float and S6-91 STP-Fill high liquid sensor failed when tested. <br /> REGULATION GUIDANCE: The interstitial space of the UST shall be maintained, as designed, under constant <br /> vacuum or pressure such that a breach in the primary or secondary containment is detected before the hazardous <br /> substance is released into the environment. <br /> CORRECTIVE ACTION: The sensors were replaced and retested during the inspection. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended <br /> and replaces the initial report. Violation#107 was removed. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
