It's needed to start a file for your proposed work. Please read the 'fine' print at the bottom and sign the form. You can re-scan it and PDF it back to me, or fax it to me at (209) 468-3433. I also attached the "Well Permit Application Form" pak (both pages). You/the driller will need to fill out BOTH pages and send it back to me before you can drill. Ideally, the C-57 driller signs it and returns it to me, but mostly consultants fill out and sign page one. If you'd like, YOU can fill out and sign page one of the form and then the C-57 driller has to fill in the bottom of page 2 and authorize your signature. Not to complicate it all, but if the driller does page one, don't worry about page two. It's contractor stuff. After all the papers are here and ok, I'll process and issue the permit. I will notify you and you may then make an appointment to do the drilling. I am to be onsite for the grouting. If all this is done efficiently, my review, field inspection and report review (after you send it to me) may/can be done with only the costs you sent in already ($374, 3-hours). If not, I will send you a bill for the time over the 3-hours. good luck.. .already, not signing the MFR is loosing on the 'efficiency' count... just kidn..your first freebie..
