Health and Safety Plan, Subsurface Investigation
Potential New Court Site, Stockton, California
HEALTH AND SAFETY PLAN APPROVAL
FOR SUBSURFACE INVESTIGATION
This Health and Safety Plan (HASP) was prepared for employees performing a limited scope of work as
defined in this HASP. It was prepared based on the best available information regarding the physical and
chemical hazards known or suspected to be present on the project site. While it is not possible to discover,
evaluate, and protect in advance against all possible hazards which may be encountered during the
completion of this project, adherence to the requirements of the HASP will significantly reduce the potential
for occupational injury.
By signing below, I acknowledge that I have reviewed and hereby approve the HASP for the work site.
This HASP has been written for the exclusive use of Earth Tech, Inc., its employees, and subcontractors.
The plan is written for specified site conditions, dates, and personnel, and must be amended if these
conditions change.
Written by:
Richard Burzinski, P.G.
Project Manager
(916) 830-4353
Approved by:
Greg Delson, P.G.
Program Manager
(858) 790-6817
Joe Bermudez, CSP
Western District Safety Manager
(562) 951-2242
Date
Date
Date
Project 104290.19.01 i January 2009
