TASK HAZARD ANALYSIS (THA)
EAR TH*4•NEr T ECH SUBSURFACE INVESTIGATION
JCC/AOC RFP 208: POTENTIAL NEW COURT SITE A trica INTERNATIONAL LTD. COWAN,
STOCKTON, CA
Evaluated by: Richard Burzinski Date: January 2009
TASK NAME
SUBSURFACE SOIL SAMPLING — DIRECT PUSH DRILLING
TASK DESCRIPTION CHEMICAL EXPOSURE HAZARDS
Using direct push technology, soil samples will be collected at selected intervals. Direct push
involves the insertion of metal "push rods" into the soil by the use of a hydraulic ram assembly.
As depths increase additional rods can be added to provide a continuous run. At the conclusion of
direct push sampling procedure the push rods are withdrawn, and the hole is sealed using grout.
Direct push "drilling" produces little to no spoils, regardless of the depths obtained (soil is
compressed to the sides of the hole rather than removed along an auger). For this reason, direct
push techniques present little potential for the airborne release of contaminants as the rods are
advanced or withdrawn.
Earth Tech will install 12 soil boreholes, 4 at Site 1 and 8 at Site 2. Each soil boring will be
advanced to a depth of approximately 10 feet bgs. Select soil samples will be collected for
analyses. Soil samples will be screened using a photoionization detector (PID).
Petroleum hydrocarbons (dermal)
BTEX (dermal)
MTBE (dermal)
Fuel Oxygenates (dermal)
Metals (dermal)
PPE OTHER SAFETY EQUIPMENT PHYSICAL HAZARDS
Level D Ensemble (SH&E 113)
Hard hat
Safety glasses
Traffic safety vest (to maintain
visibility)
Short/long- sleeved shirt
Full-length pants
Safety-toe boots
Nitrile gloves
First aid kit (located in vehicle)
Fire extinguisher (located in vehicle)
Portable eyewash station (located in
vehicle) .
Slips, trips, falls, and protruding objects
Hazardous noise environments
Drill rig hazards
Drum handling
Traffic safety
Lifting hazards
APPLICABLE OPERATIONAL SAFETY PROCEDURES ADDITIONAL SAFETY CONSIDERATIONS
Slips, Trips, Falls, and Protruding Objects (Section 6.1)
Hazardous Noise Environments (Section 6.2)
SH&E 404, Manual Lifting Procedures (Attachment B)
SH&E 405, Handling of Drums and Large Containers
(Attachment B)
SH&E 403, Drilling (Attachment B)
SH&E 517, Traffic Safety (Attachment B)
Evaluate surrounding work area for additional hazards that may be
present.
Establish responsibilities and roles of all personnel involved.
Document roles/responsibilities in log book.
The Drill Rig Inspection List in SH&E 403 must be completed prior
to starting drilling activities.
MONITORING PROCEDURES
See next page.
