(AGE) has prepared the following inform.ation for your consideration: AGE prepared a work plan in January 2001, Interim Remediation and Site Assessment Work Plan, dated 25 January 2001 was approved by the San Joaquin County - Environmental Health Department (EHD) on 16 February 2001. This plan contained procedures for site assessment and interim remediation. This work plan was sent you at your office by cover letter dated 26 February 2001, hence this plan should have been on file in your office. On 02 April 2001, Ms. Jennifer White of your office approved the cost by letter for the installation of three soil borings and installation of six deep, multi-level monitoring wells based on the work plan review. On 14 April 2001, Ms. Jennifer White of your office denied approved the cost by letter for the interim remediation of the site, stating reasons that no work plan was present, however, 12 days prior the approved of the site assessment portion of this work plan, also based on the work plan review. Therefore your office lost the work plan dated 16 February 2001. On 29 March 2002 a pre-approval request for $99,985.00 was submitted to your office. This was followed by a letter that indicated a work plan and LOP approval was not in found with the pre-approval request.
