While the NOI requests coverage under General Order <br />R5-2008-0149, coverage would actually fall under the updated Waste Discharge Requirements <br />General Order for In-Situ Groundwater Remediation and Discharge of Treated Groundwater to <br />Land (General Order R5-2015-0012). A fee check in the amount of $4,699 was received on <br />12 July 2018. <br />Prior to issuing a Notice of Applicability, and in accordance with the General Order, a mandated <br />30-day public comment period must be completed by distributing a Fact Sheet to property <br />owners/tenants within 500 feet of the Site property. As such, please submit a Draft Fact Sheet <br />for Central Valley Water Board staff review that includes the following information: <br />Site name, location, and case information. <br />The start and end dates of the public comment period. <br />A brief case summary. <br />A brief summary of the proposed remedial solution. <br />Contact information for those wishing to receive more information related to the case. <br />A figure depicting the current maximum extent of hydrocarbon impact to groundwater <br />above Water Quality Objectives. <br />Along with the draft Fact Sheet, please also submit a map showing the Site property with the <br />boundary of the 500-foot radius, and a mailing list of owners/tenants for all of the properties <br />within the boundary. Please also include the mailing address for San Joaquin County <br />Environmental Health Department, as they also must be notified of the intent to perform <br />remediation at the Site. Upon approval of the fact sheet you will be able to mail the fact sheet to <br />all applicable parcel owners/tenants. Please allow a 35-day comment period, to allow for mailing <br />delays. All comments are to be submitted directly to the Central Valley Water Board office to my <br />attention.
