STANDARD PROVISIONS AND REPORTING REQUIREMENTS -10 <br />National Pollutant Discharge Elimination System <br />b. Short-term chronic bioassays shall be performed in accordance <br />with EPA guidelines (EPA/600/4-89/001 and subsequent amendments). <br />Laboratories that perform sample analyses must be identified in all <br />monitoring reports submitted to the Board and EPA. <br />The Discharger shall conduct analysis on any sample provided by EPA <br />as part of the Discharge Monitoring Quality Assurance (DMQA) program. <br />The results of any such analysis shall be submitted to EPA's DMQA <br />manager. <br />Effluent samples shall be taken downstream of the last addition of <br />wastes to the treatment or discharge works where a representative <br />sample may be obtained prior to mixing with the receiving waters. <br />Samples shall be collected at such a point and in such a manner to <br />ensure a representative sample of the discharge. <br />All monitoring and analysis instruments and devices used by the <br />Discharger to fulfill the prescribed monitoring program shall be <br />properly maintained and calibrated as necessary, at least yearly, to <br />ensure their continued accuracy. <br />The CWA provides that any person who falsifies, tampers with, or <br />knowingly renders inaccurate any monitoring device or method required <br />to be maintained under this Order shall, upon conviction, be punished <br />by a fine of not more than $10,000 per violation, or be imprisoned <br />for not more than two years per violation, or by both. <br />The Discharger shall retain records of all monitoring information, <br />including all calibration and maintenance records, all original strip <br />chart recordings of continuous monitoring instrumentation, copies of <br />all reports required by this Order, and records of all data used to <br />complete the application for this Order. Records shall be maintained <br />for a minimum of five years from the date of the sample, measurement, <br />report, or application. This period may be extended during the course <br />of any unresolved litigation regarding this discharge or when <br />requested by the Regional Board Executive Officer. <br />The records of monitoring information shall include: <br />the date, exact place, and time of sampling or measurements, <br />the individual(s) who performed the sampling of measurements, <br />the date(s) analyses were performed, <br />the individual(s) who performed the analyses, <br />the laboratory which performed the analyses, <br />the analytical techniques or methods used, and <br />the results of such analyses.
