Box 2009 <br />Stockton, California 95201 <br />(209) 466-6781 <br />Jogi Khanna, M.D., M.P.H., District Health Officer <br />SERVING <br />City of Lodi <br />San Joaquin County <br />San Joaquin County <br />City of EscaIon <br />City of Manteca <br />City of Ripon <br />City of Stockton <br />City of Tracy <br />San Joaquin County <br />DATE: May 3, 1988 <br />TO: Unit IV Staff <br />FROM: Laurie Cotulla, Supervisor <br />RE: Implementation of Fee for Review of Environmental <br />Assessment Activities <br />Effective immediately the following will apply to activities <br />associated with hazardous waste sites, underground tank sites <br />and other environmental assessment activities. <br />A general E.H. application form must be completed and <br />a $35.00 consultation fee paid at the time of submittal <br />of work plans, proposals, reports and remedial action <br />plans (including soil treatment and/or disposal plans). <br />The fee is in addition to any permit or inspection fees <br />for monitoring wells/soil borings, tank closure plans, etc. <br />Total office and field time spent (including phone calls <br />and meetings) on these activities in excess of one hour <br />will be billed at the rate of $35.00/hour. <br />The billing statement will be sent to the applicant identi- <br />fied on the application form upon completion of the requested <br />activity, if completed within 30 days, or on a monthly basis. <br />A separate application form and fee will be required for each <br />environmental assessment activity submittal. <br />Time spent on these feed activities must be identified on <br />daily activity reports by using the "consultation activity <br />code 6. <br />An exception to this policy will be work plans which have <br />been approved by DOHS-TSCD, RWQCB and/or EPA and only re- <br />quire issuance of soil boring/well permits and the following <br />are true: <br />Written verification of other agency approval is <br />provided. * <br />Plans only requiring review for compliance with local <br />ordinances and regulations (e.g. encroachment permits <br />obtained, isolation distances, destruction and con- <br />struction methods). <br />Continued...
