Well Installation Report <br />Project No. 507.2 <br />July 3,2014 <br />May 2014 — Three groundwater monitoring wells surveyed <br />*pre 1999 data from Wm. J. Hunter & Associates documentation. <br />In January 2014, Phase II of the CAP was implemented. On January 14, 2014, three (3) <br />borings were drilled and three (3) groundwater injection wells were installed near the center <br />of the property, south and southeast of the former UST pit. <br />In March 2014, Phase HI of the CAP was implemented. Between March 24 and March 28, <br />2014, four (4) borings were drilled and four (4) groundwater injection wells were installed <br />near the center of the property, south and east of the former UST pit. On May 16, 2014, three <br />(3) borings were drilled and three (3) groundwater monitoring wells were installed up, side <br />and down-gradient of the groundwater hydrocarbon plume. <br />The following report describes the well installation activities and soil sample analyses that <br />were performed at the site. <br />2. INJECTION WELL & MONITORING WELL INSTALLATION <br />Ground Zero Analysis, Inc. installed a total of seven (7) injection wells and three (3) <br />monitoring wells during the implementation of Phases II and III of the CAP which are <br />outlined in the Phase II Corrective Action Plan Implementation work plan dated November 15, 2013 and the Phase III Corrective Action Plan Implementation work plan dated February 28, 2014. <br />The locations of the seven (7) injection and three (3) monitoring wells are shown in Figures <br />2A and 2B. <br />2.1. Injection Well Construction <br />Injection wells IW-5 thru IW-8, 1W-10 and 1W-11 and monitoring wells MW-11 thru MW-13 <br />were installed using a Boart Longyear BK-81 hollow stem auger rig operated by V&W <br />Drilling Company (C-57 License# 720904). Injection well IW-9 was installed using a low- <br />clearance hollow stem auger rig operated by Woodward Drilling Co., Inc. (C-57 License# <br />710079). <br />Injection wells IW-5, IW-6 and IW-7 were drilled with a 12-inch outside diameter, <br />continuous flight hollow stem auger. Injection wells IW-8 thru 1W-11 were drilled initially <br />with a 8-inch outside diameter, continuous flight, hollow stem auger and reamed out with a <br />15-inch auger prior to well construction. Monitoring wells MW-11 thru MW-13 were drilled <br />with an 8-inch outside diameter continuous flight auger. Pressure washing methods were <br />used to decontaminate the drilling equipment between each well. <br />Ground Zero Analysis, Inc. 3
