SAN J 0 A Q I I I Irl Environmental Health Department <br /> CCUNT <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> LOPEZ&LOPEZ AUTO REPAIR 2312 E WEBER AVE, STOCKTON May 11, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: The Hazardous Materials Inventory forms for ethylene glycol (antifreeze)were not complete.The <br /> ethylene glycol was observed in a 55-gallon drum on-site, however the chemical location is reported as"in an above <br /> ground oil disposal tank in a storage room"and the inventory storage information is reported as"aboveground tank." <br /> REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br /> of the business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS)annually by the due date, January 15. For 2019 the business plan <br /> information was submitted on 2/21/2019.The business plan information was not submitted for 2020 or 2021. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 12 HSC 25505(c)Failed to have the HMBP readily available to facility or EHD personnel. <br /> OBSERVATION: Cristobal Lopez, owner, could not provide the emergency response plans and procedures, <br /> chemical inventory, and site map, upon request. <br /> REGULATION GUIDANCE: Emergency response plans and procedures, chemical inventory, and site map shall be <br /> readily available to personnel of the business or facility with responsibilities for emergency response or training. <br /> CORRECTIVE ACTION: Immediately make all this content available on site, either electronically or in hard copy <br /> format. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were 3 violations during the <br /> inspection.
