The following was noted: <br />• The rentention pond was approximately 8 ft deep, required setback to septic system is 100 ft. <br />• The proposed septic system area was east of the retention pond, west of the restroom building, and north of the office. <br />• It was 67 ft from the edge of the office to the retention pond and 102 ft from the edge of the restroom building to the pond. <br />• It was 88 ft from the retention pond to the east fence which is the furthest point where the septic system was proposed. <br />• There was a monitoring well located north of the restroom building and 98 ft east of the retention pond. <br />• The edge of the proposed septic system area to the monitoring well is 10 ft, required setback is 100 ft. <br />• Proposed area for the septic system does not meet setback requirements to the retention pond and monitoring well. <br />• Existing septic tank and pipes after tank were damaged. The top of the tank was partially collapsed and some of the pipes <br />were flattened according to Daniel. The tank was covered with two steel plates and the pipes were replaced. <br />• The clean out for the office building was open. <br />Corrective actions: <br />• Find area that would meet septic system setback requirements. <br />• Obtain septic tank destruction permit from EHD and destroy damaged septic tank. <br />• Cap office building clean out. <br />RECEIVED BY: EMAILED <br />ENVIRONMENTAL HEALTH SPECIALIST: DARIA AFONSKAIA DATE: OCTOBER 26, 2020 <br />EHD 48-037 Official Consultation Repoli <br />9/18/07 <br />
