OBSERVATION: The designated operator on 10/26/2021 failed to complet a written report to document the inspection findings on the new and approved report format. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the new and approved report format. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the approved reports fprmat. This is a minor violation. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificate dated 01/02/2021 did not include the initial training dates, and dates of employee assuming responsibility as a facility employee. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation. 306 HSC 25291(e)Failure to remove, analyze, and properly dispose of water in secondary containment(pre-Jul 2003). OBSERVATION: Liquid was observed in the two 87 fill sumps and 91 fill sump. REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. CORRECTIVE ACTION: The service technician removed approximately 1.5 gallon of liquid from the two 87and 91 fill sumps. Ensure that all sumps and annular spaces are maintained free of liquid. This was corrected on site. This is a repeat violation, Class ll. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by July 16, 2021 .
