Time In: 12:01 pm <br /> r <br /> Time Out: 12:27 pm <br /> Food Program Complaint Inspection Report <br /> Name of Facility: RAW JAPANESE BISTRO&SUSHI BAR Date: 05/26/2021 <br /> Address: 10742 TRINITY PKWY, STOCKTON 95219 <br /> Owner/Operator: LE, HOANG D Telephone: (209)954-9729 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00053803 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES THERE IS NO HAND WASHING, THE RESTROOM WAS FILTHY, NO HAND WASHING <br /> STATION WHERE SUSHI IS PREPARED, NO VISIBLE HAND WASHING OR SANITIZING OF TABLES AFTER USAGE, <br /> SEAFOOD IS NOT FRESH POSSIBLE TTC VIOLATION OR NO ADHERENCE.VISIBLE BACK KITCHEN FLOOR DIRTY, <br /> FOOD LEFT OUT FOR A LONG TIME BEFORE PICK UP FOR TO GO ORDERS. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I spoke to the manager who was not aware of complaint prior to this investigation. <br /> I observed both the Men's and Women's restrooms,which does not appear to be"filthy". <br /> I observed 2 hand wash sinks in to proximity of sushi station. <br /> One hand sink lacks soap. <br /> Paper towels are not in a dispenser. <br /> Provide paper towels in a dispenser. <br /> The other hand sink(near sushi bar)is obstructed by disposable face masks. <br /> Remove face masks so that hand sink is accesble and unobstructed. <br /> Corrected onsite. <br /> I observed glove use by sushi chefs. <br /> I discussed proper glove use/hand washing with the manager. <br /> I observed employee hand a"to go" bag to a 3rd party delivery service person, and then handle tableware(plates and <br /> wrapped silverware). <br /> Wash hands when handling clean dishes and utensils. <br /> Tables are currently occupied and I did not observe how/if tables are sanitized.I observed seafood at sushi station. <br /> Seafood is stored in the sushi display cases. <br /> I observed debris on the kitchen flooring under prep sink and near water heater. <br /> Clean <br /> I observed 2"to go"orders picked up by 3 rd party delivery service workers. <br /> "To go"orders were being prepared as the 3rd party delivery service workers waited for orders. <br /> I did not observe"food left out for a long time for to go orders" <br /> A chargeable reinspection to occur on or after 6/9/2021 to follow up on items noted on this report. <br /> FA0017966 C00053803 SCO04 05/26/2021 <br /> EHD 16-23 Rev.06/30/15 Page 1 of 2 Food Program Complaint Inspection Report <br /> 1868 E.
