Time In: 1:40 pm <br /> Time Out: 2:11 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: RAW JAPANESE BISTRO&SUSHI BAR Date: 09/15/2021 <br /> Address: 10742 TRINITY PKWY, STOCKTON 95219 <br /> Owner/Operator: LE, HOANG D Telephone: (209)954-9729 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00054220 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES THE FACILITY USES IMPROPER THAWING TECHNIQUES,THE HAND SINK IS <br /> OBSTRUCTED, FOOD ITEMS ARE STORED ON THE FLOOR,AND THERE IS A LACK OF FOOD MANAGER AND <br /> HANDLER CERTIFICATES. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I went to the facility and employee was aware of compliant prior to my inspection. <br /> I observed table in front of hand sink. <br /> I spoke to Person In Change over the phone, per person in charge the hand sink is obstructed but there is another hand sink <br /> on the other side of the sushi bar. <br /> I let him know that the hand sink to remain unobstructed. <br /> Hand sink was made accessible during inspection. <br /> I did not observe thawing at the time of inspection but I did speak to employees who explained the proper process. <br /> Person In Charge is to email Food Manager and food handler certificates by the end of the week. <br /> I observed food that is on the floor, however per employees shipment came today and food will be stocked today. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.If a reinspection is required,fees will be assessed at the current hourly rate. <br /> Received by: Name and Title: Alex, <br /> EH Specialist: STEPHANIE RAMIREZ Phone: <br /> FA0017966 C00054220 SCO04 09/15/2021 <br /> EHD 16-23 Rev.06/30/15 Page 1 of 1 Food Program Complaint Inspection Report <br /> 1868 E.
