Health and Safety Plan
453 Moffat Blvd
Manteca, California
11.0 FIRST AID AND MEDICAL TREATMENT
All persons on site must report any near-miss incident, accident, injury, or illness to the SSHO. A
trained site first aid provider will provide first aid. Injuries and illnesses requiring medical
treatment must be documented. The SSHO must conduct an accident investigation as soon as
emergency conditions no longer exist and first aid and/or medical treatment have been
completed. All necessary reports must be completed and submitted to the Project Manager
within 24 hours after the incident.
If treatment beyond first aid is required, the injured should be transported to the nearest
medical facility off-site. If the injured is not ambulatory, or shows any sign of not being in a
comfortable and stable condition for transport, then an ambulance/paramedics should be
summoned. If there is any doubt as to the injured worker's condition, it is best to let the local
paramedic or ambulance service examine and transport the worker.
The closest medical facility is the Doctors Hospital of Manteca, 1205 E North St, Manteca,
California 95336. The emergency room phone number is (209) 823-3111. A map of the driving
directions is included in Appendix D.
Directions (1.4 miles, 5 minutes):
Head northwest on Moffat Blvd toward S Lincoln Ave (56 ft)
Turn right onto S Lincoln Ave (0.5 miles)
Turn right onto E North St (0.9 miles)
1205 E North St is on the left.
Terraphase Engineering Inc.
