Due to the historical concentration <br />trends seen in the sentinel groundwater wells, and as noted in the recent CVRWQCB <br />correspondence, additional delineation of dissolved impacts in the A and B zones is warranted. <br />Table 2 contains the historical groundwater monitoring and analytical data and Table 3 <br />summarizes the site-related soil boring and well construction details. <br />For a full background historical site activities see Appendix B. <br /> <br />3.0 Proposed Scope of Work <br />Stantec plans to install three groundwater monitoring wells as detailed below to enhance the <br />delineation of the identified 'A' and 'B' hydrogeologic zones. <br /> <br />3.1 SITE HEALTH AND SAFETY PLAN <br />As required by the Occupational Safety and Health Administration Standard "Hazardous Waste <br />Operations and Emergency Response" guidelines (29 CFR 1910.120), and by the California <br />Occupational Health and Safety Administration "Hazardous Waste Operations and Emergency <br />Response" guidelines (CCR Title 8, Section 5192), Stantec will create a site-specific Health and <br />Safety Plan (HASP) prior to the commencement of field work. The HASP will outline potential <br />hazards to Stantec field personnel during the field activities described herein. Job safety <br />analyses for tasks to be performed by Stantec personnel (e.g., driving, soil gas well installation, <br />vapor sample collection, etc.) will be included. The HASP will also include required personal <br />protective equipment to be worn by Stantec field personnel for each task. In addition, Stantec <br />will produce a Journey Management Plan (JMP) to prevent losses associated with motor vehicle <br />incidents. A copy of Stantec's HASP and JMP will be distributed to subcontractors prior to field <br />work, and will be available on-site during field activities. <br />Subcontractors will also develop a site-specific HASP and JMP for tasks applicable to their scope <br />of work (e.g., driving, air-knifing, advancing soil borings, etc.). Stantec will review subcontractor <br />HASPs prior to commencing field activities. Appropriate subcontractor HASPs will also be <br />available on-site. <br /> <br />3.2 PERMITTING AND NOTIFICATION <br />Prior to initiating any field work, Stantec will apply for a subsurface drilling permit from San <br />Joaquin County Environmental Health Department (SJCEHD) for the installation of groundwater <br />monitoring wells MW-15, MW-16B, and MW17B (Figure 3). Notification will be provided to the <br />SJCEHD no less than 48 hours prior to the initiation of field work, and to the property owners in <br />accordance with the access agreement. Stantec will also obtain an encroachment permit from <br />the city of Stockton for the wells planned to be installed in the city right of way. <br />Stantec <br />3
