Fall Hazards <br />Fall hazards present. Working at a height <br />of 6' or more above a lower surface or <br />working above dangerous equipment <br />x Personal Fall Arrest System designed or <br />approved by a qualified person <br />Water Hazards <br />Working on or above water where <br />drowning hazards exist <br />x U.S. Coast Guard approved personal <br />flotation device - Type I, II, or III PFD <br />Heat, Flame, or Flash Fire or Arc Hazards <br />Working in extreme heat x Consider use of a cooling vest <br />Welding activities, hot work x Welding leather clothing or Fire-Resistant <br />Clothing (FRC) <br />Performing hot work (e.g., cutting, <br />welding, brazing or grinding) on pipe <br />and vessels (e.g., tanks, KO drums, <br />separators) that contain or previously <br />contained hydrocarbon fuels <br />x FRC <br />Fueling/refueling equipment where <br />engineering controls do not adequately <br />prevent the risk of flash fires <br />x FRC <br />Working at active pipeline sites x FRC; ensure compliance with Chevron Pipe <br />Line FRC requirements <br />Working in a refinery x Adhere to the refinery's FRC requirements <br />FRC Hazard Analysis for Upstream Business Unit teams -- in determining the need for FRC, research the <br />history of the well where work will be performed and consider the type of well (e.g., gas, oil, low <br />pressure, high pressure) and the type of work operations or activities to be conducted: <br />Working in an opco-operated oil field x Adhere to the opco's FRC requirements. <br />Hot work or use of an open flame in an <br />oil field or in an area where flammable <br />materials may be released or <br />accumulate <br />x FRC <br />Initial rig up and normal drilling or <br />workover operations prior to reaching <br />active hydrocarbon zones <br />x FRC is not typically required unless other <br />activities are being conducted in close <br />proximity (e.g., fracing a previously drilled <br />well while rigging a well in close proximity); <br />workers must don FRC prior to reaching the <br />hydrocarbon zone. "Gas or hydrocarbon <br />zones" are areas/zones where prior history <br />or current well conditions indicate that <br />fluids or gas have been encountered (i.e., <br />the drilling operation experiences a well <br />kick or any other sign that the well will flow). <br />Drilling or workover activity in active gas <br />or hydrocarbon zones <br />x FRC until the final casing is cemented and <br />the well is effectively closed <br />Drilling or workover activity being <br />conduct in an area where there is a <br />history or fluid or gas kicks from <br />underground producing zones <br />x FRC until the final casing is cemented and <br />the well is effectively closed <br />Pulling wet string tubing x FRC
