SITE-SPECIFIC HEALTH AND SAFETY PLAN FOR WELL INSTALLATIONS <br />Site Control and Safety Procedures <br />May 4, 2017 <br />triangles in that direction to provide passing traffic with ample warning. <br />Recommended placement of warning devices shall be no less than 100 feet or <br />more than 500 feet from the disabled vehicle. <br />If the vehicle becomes disabled on a divided or one-way highway, the <br />recommended placement of warning devices shall be so that the first is 200 feet <br />to the rear, the second is 100 feet to the rear, and the third is not more than ten <br />feet to the rear. Warning devices should be placed less than two feet to the <br />roadside of the widest portion of the vehicle or load. <br />If the vehicle becomes disabled (not including a flat tire), a professional <br />emergency roadside assistance company will be contacted to tow or enable <br />the vehicle. If such company is not available, proper towing equipment (tow <br />strap, cables or other equipment specifically designed for towing) must be used. <br />Drug and Alcohol Testing <br />Following an incident, Stantec will follow the incident reporting procedures. If appropriate, <br />Stantec will include drug and alcohol testing, consistent with Stantec's Policies and Procedures, <br />as well as the client's contractual requirements for testing. <br />Exclusion Zone and Decontamination <br />No eating, drinking, or smoking within the exclusion zone. These activities will be conducted only <br />in designated areas of the site. Use of hand held electrical devices, including cell phones (with <br />the exception of intrinsically safe devices) are prohibited in the exclusion zone. Personnel will <br />properly decontaminate after leaving an exclusion zone. Decontamination procedures may <br />involve disposing of Tyveks, latex gloves, etc. in a decontamination zone located immediately <br />outside of the exclusion zone. At a minimum, personnel will wash any exposed skin before <br />leaving a site using soap and water or pre-moistened cleansing towels. Stantec will evaluate <br />the hazards and develop site-specific decontamination procedures to address the chemical <br />hazards at each site. These procedures can be found in the JSAs in Attachment 5. <br />Fit for Duty <br />Safety is influenced by many factors, but the most important is the health and well-being of <br />Stantec's employees and partners. Physical and mental health is just as important as good <br />tools, good practices, and good job planning. <br />These questions are designed to help you do a quick self-assessment of your physical and <br />mental health. Any concerns resulting from your assessment regarding your ability to carry out <br />your job responsibilities safely and in good health need to be discussed with your supervisor <br />before starting work. <br />Am I feeling good today and ready to work at my typical level of physical activity and <br />responsibility? <br />Do I have any sprains, strains, areas of weakness, or soreness? <br />Am I managing multiple areas of stress? <br />Am I well hydrated? <br />Any physically demanding activities recently (chores, sports, hobbies)? <br />Am I well rested with a good energy level? When did I eat last? <br />Am I taking any medications that can make me drowsy or adversely affect my safe <br />performance? <br />Any cuts or scrapes are clean and bandaged? <br />37
