SITE-SPECIFIC HEALTH AND SAFETY PLAN FOR WELL INSTALLATIONS <br />Emergency Response <br />May 4, 2017 <br />1.5 EMERGENCY RESPONSE PROCEDURES: INJURY OR ILLNESS <br />If an injury or illness occurs, take the following action: <br />Stop Work, stabilize the situation, and secure the site. <br />Administer First Aid for the person immediately using a first aid and blood-borne <br />pathogens kit. <br />Determine if emergency response (fire/ambulance) is necessary. If so, call 911 or the <br />appropriate emergency response number(s) listed in Section 1.1 on the closest available <br />phone. Provide the location of the injured person and other details as requested. <br />If emergency decontamination is required: <br />Immediately remove any contaminated personal protective equipment (PPE) or <br />clothing. <br />If possible, wash contaminated area with mild soap and water. <br />Personnel assisting the contaminated individual will don the proper PPE to avoid <br />unnecessary exposures. <br />For all injuries or illness, even minor cuts, scratches, and bruises, notify the SHSO <br />immediately. <br />After the victim(s)/site have been stabilized, the SHSO is responsible for notifying the <br />Stantec Project Manager. The Stantec Project Manager will initiate appropriate incident <br />reporting procedures as described in Section 1.7. <br />The SHSO will assume responsibility during a medical emergency until more qualified <br />emergency response personnel arrive at the site as needed. <br />1.5.1 Injury or Illness Requiring Hospital/Clinic Service WITHOUT Ambulance <br />Service <br />For injuries or illnesses requiring medical care from a hospital or occupational clinic without <br />requiring ambulance services, the following procedures will be taken immediately: <br />The SHSO will ensure prompt transportation of the injured person to a hospital or <br />occupational clinic and make immediate notification to the Stantec PM. The Stantec <br />Project Manager will initiate appropriate incident reporting procedures as described in <br />Section 1.7. <br />As applicable, the Stantec HES Team will facilitate communication between the injured <br />person and Stantec subscribed service for occupational injuries (AllOne Health) <br />A representative of Stantec will accompany the injured person and remain at the <br />medical facility until the employee is ready to return. <br />If the driver of the vehicle is not familiar with directions to the hospital, a second person <br />shall accompany the driver and the injured employee to navigate the route to the <br />hospital. <br />If it is necessary for the SHSO to accompany the injured employee, provisions will be <br />made to have another employee, properly trained and certified in First Aid, to act as the <br />temporary SHSO. <br />If the injured employee is able to return to the job site the same day, he/she will bring <br />with him/her a statement from the doctor containing such information as: <br />Date <br />Employee's name <br />Diagnosis <br />Stantec <br />7
