DATA GAP ASSESSMENT WORK PLAN AND PATH TO CLOSURE REVIEW <br />Chase Chevrolet, 424 N. Van Buren Street <br />Proposed Scope of Work <br />January 2, 2018 <br />4.0 PROPOSED SCOPE OF WORK <br />To close the data gaps under SWRCB LTCP, Stantec proposes to advance two direct push soil <br />borings at onsite locations (SB-1 and SB-2) to collect shallow soil and grab groundwater samples, <br />install three shallow soil vapor probes (SV-1 through SV-3), and perform an updated sensitive <br />receptor survey within 1,000 feet of the site. <br />A summary of the proposed scope of work follows below. Proposed assessment locations are <br />illustrated on Figure 2 and Stantec's field procedures are included in Appendix E. <br /> <br />4.1 SITE HEALTH AND SAFETY PLAN <br />As required by the Occupational Safety and Health Administration Standard "Hazardous Waste <br />Operations and Emergency Response" guidelines (29 CFR 1910.120), and by the California <br />Occupational Health and Safety Administration "Hazardous Waste Operations and Emergency <br />Response" guidelines (CCR Title 8, Section 5192), Stantec will create a site-specific Health and <br />Safety Plan (HASP) prior to the commencement of field work. The HASP will outline potential <br />hazards to Stantec field personnel during the field activities described herein. Job safety <br />analyses for tasks to be performed by Stantec personnel (e.g., driving, soil sampling, air <br />monitoring, etc.) will be included. The HASP will also include required personal protective <br />equipment to be worn by Stantec field personnel for each task. In addition, Stantec will <br />produce a Journey Management Plan (JMP) to prevent losses associated with motor vehicle <br />incidents. A copy of Stantec's HASP and JMP will be distributed to subcontractors prior to field <br />work, and will be available on-site during field activities. <br /> <br />4.2 PERMITTING AND NOTIFICATIONS <br />Stantec will obtain a monitoring well and soil boring installation permit, or permits as necessary, <br />from SJCEHD. Additionally, an encroachment permit from the City of Stockton Community <br />Development Department (CDD) will be obtained for the advancement of boring SB-2. Proper <br />notification to SJCEDH and CDD per the terms of the permits will be provided prior to the <br />execution of field work. <br /> <br />4.3 SUBSURFACE UTILITY CLEARANCE <br />Prior to performing subsurface disturbance for the collection of soil and groundwater samples, <br />Stantec will mark proposed assessment locations with white paint in accordance with <br />Underground Services Alert North (USA) guidelines. Stantec will contact USA North at least 3 <br />days prior to the start of field work to allow USA members to mark subsurface utility locations. <br />Additionally, Stantec will contract a private utility locator to verify the locations of marked USA <br />member utility lines and investigate the presence of utility lines that are not owned or operated <br />by USA members.
