The following is an itemized list of hazardous materials violations that have not been addressed for GRANT LINE SHELL* as of July 22, 2021. Open violations from May 26, 2021 inspection Violation#6-Failed to electronically submit a site map with all required content. OBSERVATION: The site map is incomplete. The following items are missing from the site map: • Carbon dioxide loading area • Fuel loading area • Internal roads • Map Symbol definition for the 4 triangles shown surrounding the USTs • Additional emergency response equipment/absorbents were found in the back storeroom of the store that were not listed on the map The following items are inaccurate on the map: • Waste absorbent and fuel filter drums were not found at the side of the store;they were found in the storage room next to the car wash REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#11 -Failed to provide or document initial and/or refresher training to appropriate personnel. OBSERVATION: Training records for all employees could not be located on site during the inspection.Anthony Brown and Donte Mcclain Mitchell are new employees that have not yet been trained. REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training.All employees shall be trained in safety procedures in the event of a release or threatened release of a hazardous material, including, but not limited to,familiarity with the following plans and procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified program agency. (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment. (C) Evacuation plans and procedures, including immediate notice,for the business site. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years.
