The Business Phone number <br /> is reported as 650-906-1264 vs. 209-931-4619. <br /> REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calep;a.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> BSERVATION: The Hazardous Materials Inventory forms for carbon dioxide were not complete. The storage <br /> pressure is wrongly reported as "ambient' vs. "above ambient' , <br /> REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br /> of the business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. The site map does not show/address the following: <br /> Internal roads (none at this facility). <br /> 2. Evacuation staging area. <br /> 3.
