Violations 122, 1ZS, 204, and $12 <br />r <br />EN T <br />These violations pertain to the inappropriate disposal of hazardous waste; laek of records/manifests <br />demonstrating appropriate disposal, and failure to maintain manifests for three years. <br />CORRECTIVE ACTION: Manifests were reviewed for 1240 to ensure the store had copies of <br />manifests on site. Our vendor StrongPak submits fully executed manifests to DTSC on behalf of all <br />California Dollar Tree stores within the 30 -day timeframe required. The manifests are sent via USPS <br />or Fed Ex, with tracking numbers, so that they are able to confirm date of receipt at DTSC. <br />Please see the following: <br />A1. Violation X03 <br />This violation pertained to the lack of a modified contingency response plan and contact information <br />for emergency coordinator(s). <br />CORRECTIVE ACT10N: This facility's current Emergency f2espanseJContingency Plan was accepted <br />by the CUPA on Feb 21, 2014, under CERs No. 10406311. This emergency plan with contact <br />information has been posted next to the store's phone. In addition, this facility has posted the <br />completed "Hazardous Waste Emergency Information" form provided during your inspection. <br />Please see B. Violation 703 (Employee Trainingj <br />This violation pertains to lack of training for facility employees on hazardous waste handling and <br />emergency procedures. <br />CORRECTIVE ACTION: All store employees received hazardous waste training beginning in <br />November 2013, with refresher training for pertinent employees in June 2014, Employees <br />completed either the Company's online training or review of the hardcopy training guide. Dollar Tree 1240 Hazardous Materials Training Sienoffs <br />CORRECTIVE ACTION: The Store Management team received classroom -based refresher training on <br />May 8, 201.4, during which they reviewed key policies and procedures for managing the hazardous <br />waste program.
