RUVALCABA <br />Contacted Juan Armendarez with Caltrans (209) 483-3088. Juan stated that at <br />approximately 5:55 am a big rig hit some road debris while traveling south bound <br />on 1-5. The debris punctured the trucks fuel tank, causing a leak. The truck pulled <br />over on the HWY 33 off ramp. Juan stated that Patriot environmental was onsite <br />and had applied absorbent to the asphalt. The absorbent was to be swept to the <br />shoulder where the contaminated soil was. The contaminated soil was to be <br />excavated at a later date, once the Underground Service Alert (USA) survey had <br />been conducted. Juan provided the name of the driver as Chuong Nguyen and the <br />insurance phone number as (503) 393-7030. May Trucking is a self insured entity. <br />Juan stated that Patriot Environmental was contacted by NWFF environmental. <br />The contact for NWFF was given as Jason True (551) 231-2620. <br />Departed to site, with Lead Senior Leyna Huynh, at approximately 9:20 am. <br />Caltrans and environmental cleanup firm had departed from site by the time of <br />arrival. Based on staining and debris on road it was evident that absorbent had <br />been placed on the asphalt and swept up. Per previous conversation with Caltrans <br />absorbent was to be swept to shoulder and collected when excavation of soil was <br />complete. A portion of the shoulder was covered with a plastic like sheet and <br />weighted down with sand bags. Plastic sheets may be covering contaminated soil, <br />was unable to verify. Arrived at office at approximately 10:30 am. <br />Created photo log, Notification of Hazardous Waste Discharge, site map and <br />collected missing information. <br />Called May Trucking (503) 393-7030 and was transferred to Teresa Fisher, <br />adjuster with May Trucking, extension 13025. Teresa stated that the claim number <br />for the incident was 18-744. Email provided to send notice to abate was <br />teresa&maytrucking.com. Billing address was provided and entered into envision <br />connect. Teresa stated that North West Fire Fighters would be in charge of the <br />cleanup and that disposal records could be obtained from them. Contact info for <br />North West Fire Fighters was given as (800) 942-4614 or (541) 929-4884. <br />Awaiting USA to be performed and for cleanup to conclude.
