The test <br />results for overfill prevention testing conducted 8/29/18 were dated 8/29/13. These results incorrectly indicate the <br />flapper is located at 90%. Based on the measurements included in the results (83.7" flapper location) and the tank <br />charts available on site (11,627 gallon capacity), the flapper is set at between 96% and 97% capacity. The results <br />state the tank diameter is 93" while the tank chart states 92". Immediately repair or replace this component by a <br />properly licensed, trained, and certified contractor under permit and inspection of the EHD. If the component can't be <br />replaced immediately, there is a possibility that the jet fuel UST system may be red tagged to prevent fuel inputs. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by January 20, 2019. <br />Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
