The most recent financial <br /> responsibility documents submitted in CERS are dated 2/14/17. Current financial responsibility documents are <br /> required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, and upload the required documents. <br /> This is a repeat violation, Class II. <br /> 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. <br /> Maintenance and monitoring records for the last three years were not found on site. 2018 test results were not found <br /> on site, including monitoring system certification (1/16/18), secondary containment testing (1/16/18), and overfill <br /> prevention testing (11/9/18). These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; <br /> (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all <br /> readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br /> inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for <br /> the last three years on site and submit copies to the EHD. <br /> This is a repeat violation, Class II. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> Seeping fuel was observed on piping under dispenser 1/2. All primary containment for the UST system must be tight. <br /> Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under <br /> permit and inspection of the EHD. <br /> This is a repeat violation, Class II. <br /> 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> Overfill prevention testing was conducted on 11/9/18. This testing was required to be conducted by October 13, 2018. <br /> Ensure all future testing is conducted within required time frames. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by February 2, 2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
