The following is an itemized list of hazardous materials violations that have not been addressed for AISLE 1 #2356 as of August 25 , 2021. Open violations from July 07 , 2021 inspection Violation #6 - Failed to electronically submit a site map with all required content. OBSERVATION: The site map is inaccurate . The map is missing the following : -a storm drain was observed on the southeast corner of the dispenser island -a fire extinguisher was observed in the utility room REGULATION GUIDANCE: Asite map shall contain a north orientation , loading areas , internal roads , adjacent streets , storm and sewer drains , access and exit points , emergency shutoffs , evacuation staging areas , hazardous material handling and storage areas , and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as " NA" . CORRECTIVE ACTION : Immediately log into the California Environmental Reporting System ( CERS ) at http : //cers. calepa . ca , gov/, upload the correct or updated information , and submit to the EHD for approval . ❑ This violation was corrected ❑ This violation will be corrected by ( date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel . OBSERVATION The business plan training records were not found on site . REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training . All employees shall be trained in safety procedures in the event of a release or threatened release of a hazardous material , including , but not limited to , familiarity with the following plans and procedures : (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified program agency . ( B ) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons , property , or the environment ( C ) Evacuation plans and procedures , including immediate notice , for the business site. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years .
