Page 1, Part 2.A — Name of facility: North County Landfill (see Permit). <br />2. Page 1, Part 2.B.1- Physical address: 17900 East Harney Lane (see Permit). <br />3. Page 1, Part 2.E — Add the following permitted wastes: agricultural, compostable material, <br />construction/demolition, industrial, tires, and other (household hazardous waste, green/wood waste, yard <br />clippings). <br />4. Page 2, Part 3.B.l.a & d- Information for this section appears to be missing. <br />5. Page 2, Part 3.B.3.f & g- Clarify dates. <br />6. Page 2, Part 3.B.3. j & k- Correct numbers based on JTD revised on March 2005 or provide new <br />documents to EHD. <br />7. Page 3, Part 5.A- SCH# for Negative Declaration/Mitigated Negative Declaration appears to be <br />missing. <br />8. Page 3, Part 6.A- Change the date for JTD to March 2005. <br />SCH# for Environmental Document (EIR) appears to be missing. <br />Date for Local USE/Planning Permits needs to be change to 06/1989 (UP -89-2). <br />Missing checkmark for addendum dated Oct.90. <br />9. Page 3, Part 6.B- Indicate preliminary date for Closure/Post Closure Maintenance Plan (March <br />2005). <br />EHD did not receive the Aerial Survey dated 3/15/07. Submit Aerial Survey to EHD for review. <br />10. Page 3, Part 6.C- Date for Report of Waste Discharge needs to be change to 12/06/02 (WDR# RS - <br />2002 -0219). <br />Public Works needs to submit to EHD updated SJVAPCD Permit (EHD has a copy of Permit which <br />expired on 11/30/06). <br />No Department of Health Services Permit needed. <br />11. Page 4, Part 8- Information for facility operator name and address where legal notice may be served <br />appears to be missing. <br />Please, submit to the EHD corrections that address the above noted items.
