Also <br />find attached the Standard Observations for the inspection, as required by Section 5a. <br />This inspection was completed September 29, 2008. No serious problems were noted, but several <br />items required improvement prior to the winter rains, including: <br />• Placement of straw and grass seed on portions of the western slopes of Module 4, and top <br />deck areas of Module 1 and 3 (Figure 1 and Photo 1). <br />• Placement of anti -erosion mats and down drains on portions of the southern slopes of <br />Module 4 (Photos 2 and 3). <br />This work was in accordance with our Winterization Plan, most of which was completed by <br />October 31, 2008. <br />I am aware that there are significant penalties for submitting false information, including the possibility <br />of fine and imprisonment.
