SAN J OA U I Environmental Health Department <br /> COUNTY- AMENDED <br /> 7/19/2021 <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Panda Express#1313 4339 MORADA LN, STOCKTON July 19, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> OBSERVATION: Employee training records have not been provided. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training. All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material, including, but not limited to, familiarity with the following <br /> plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice, for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> This is a repeat violation, Class ll. <br /> 12 HSC 25505(c) Failed to have the HMBP readily available to facility or EHD personnel. <br /> OBSERVATION: Employees do not have access to the Emergency Response Plan and the inventory information <br /> on-site. <br /> REGULATION GUIDANCE: The emergency response plans and procedures, the inventory of information required <br /> by this article, and the site map required by this section shall be readily available to personnel of the business or the <br /> unified program facility with responsibilities for emergency response or training pursuant to this section. <br /> CORRECTIVE ACTION: Immediately make arrangements to have the business plan readily available for review. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on July 19, 2021. Later the same day, the report was <br /> amended by the EHD. This amended report replaces the initial report. <br /> The report was amended to add violation 12 and to remove "Consent For: Inspection and Photos and Consent <br /> Given By: Juan Martinez, Lead Cook" information and change it to "Not Applicable". <br /> Conducted in-office Hazardous Materials Business Plans (HMBP) inspection. The food inspector, Stephanie <br /> Ramirez, performed an on-site inspection for the CO2 tank on January 27, 2021. There were 8 violations during <br /> the in-office inspection.
