The column titled "Initial Training Dates" has the <br />date of the training listed instead of the employee's initial date of training. Date of training belongs in the bottom right <br />box next to the designated operator's signature. <br />REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if <br />after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training <br />Certificate shall contain accurate information for all employees trained. <br />CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br />submit a copy of the updated form to the EHD. <br />This is a minor violation. <br />302 <br />CCR 2630(d), 2641(a) Leak detection equipment not installed/maintained/located to detect leaks at earliest <br />opportunity. <br />OBSERVATION: The sensors in UDCs 1/2, 3/4, 7/8, 9/10 are not located at the lowest point of the UDC and not <br />located to detect a leak at the earliest opportunity. <br />- UDC 1/2, 3/4 have barriers from previous float and chains to contain leaks between the products. The barriers are <br />still in place with no openings to provide flow to the sensor. If a leak were to occur in the containments that the <br />sensor is not located in then the leak would not be detected due to the barrier preventing flow to the sensor. <br />- UDC 7/8, 9/10 have three open cups creating multiple low points. Each UDCs only have one sensor. The sensor is <br />located in one of the three open cups. Multiple cup openings with only one sensor in one of the cups does not allow <br />for leak detection at the earliest opportunity. <br />REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br />possible opportunity. <br />CORRECTIVE ACTION: Immediately relocate the sensors to the lowest point of UDC 12/2, 3/4, 7/8, 9/10 to allow <br />for leak detection at the earliest opportunity. <br />This is a repeat violation, Class II. <br />317 <br />CCR 2636(f)(2) Failure of the line leak detector to detect a 3.0 gph leak at 10 psi and restrict or shut off flow. <br />OBSERVATION: The 87 and 89 VMI line leak detector failed to detect a leak when tested. <br />REGULATION GUIDANCE: All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. <br />CORRECTIVE ACTION: The service technician adjusted the leak detector, retested it, and it passed. <br />This is a repeat violation, Class ll. <br />FA0001570 PR0231126 SCO01 08/05/2021 <br />EHD 23-01 Rev. 9/16/2020 Page 4 of 5 Underground Storage Tank Program OIR <br />1868 E.
