Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. All new addenda supersede the previous ones. <br />3. Provide written notification to ust(c_slgov.orq to arrange the repairs and testing (a 48-hour <br />notice is required). <br />a. Removal of barriers and installation of Bravo sensor cups and Bravo retrofit kit within UDCs <br />1/2, 3/4, 5/6, 7/8. Ensure potential leaks drain to one low point toward UDC sensor. <br />4. Any individual performing the work of a service technician must meet the following <br />qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible valid, <br />current contractor's licenses issued by the Contractors State License Board (may be issued <br />to the installation company), or a Tank Tester's license issued to the individual by the State <br />Water Board <br />b. Possess or work under the direct and personal supervision of an individual physically <br />present at the work site who possesses a valid, current California UST Service Technician <br />certificate issued by the International Code Council (ICC) <br />c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced <br />5. The service company must provide the following documentation to the tank owner/operator for <br />an inspector to review during the annual UST inspection: <br />a. Description of part(s) installed (parts list, cut sheets, etc.) <br />b. Name & UST ICC # of the service technician performing the work (ICC, manufacturer <br />certification proof, etc.) <br />c.
