THIS IS <br /> THE SECOND TIME AT THE SAME LOCATION WITH THE SAME LADY MAKING THE SALAD THIS HAS HAPPENED. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. She stated that customer informed her of problem 2 hours after he ate <br /> the salad. Salad came with chopped chicken (it was heated), spinach,tomatoes, cucumber, bell pepper, onions, pickles, <br /> banana peppers,jalapeno pepper, sweet onions, red vine vinegar, honey mustard dressing, and sweet onion pepper.
