The following is an itemized list of hazardous materials violations that have not been <br /> addressed for TRACY 76 as of September 02 , 2021 . <br /> Open violations from July 21 , 2021 inspection <br /> Violation #11 - Failed to provide or document initial and /or refresher training to appropriate personnel . <br /> OBSERVATIOR Annual training records were not available during the inspection . <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training . All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material , including , but not limited to , familiarity with the following <br /> plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency . <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons , property, or the environment <br /> (C) Evacuation plans and procedures , including immediate notice , for the business site . <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years . <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> trainingcords to the EHD , <br /> s violation was corrected ❑ This violation will be corrected by (date) : <br /> ❑ Supporting documents included r <br /> Describe actions taken or wi be taken to correct v'olation : <br /> Pji <br /> i , , <br /> • 11 - <br /> Violation M - Failed to have the HMBP readily available to facility or EHD personnel. <br /> OBSERVATION Facility could not provide the Emergency Response Plan and the inventory information upon <br /> request. <br /> REGULATION GUIDANCE: The emergency response plans and procedures, the inventory of information required <br /> by this article, and the site map required by this section shall be readily available to personnel of the business or the <br /> unified program facility with responsibilities for emergency response or training pursuant to this section . <br /> CORRECTIVE ACTION: Immediately make arrangements to have the business plan readily available for review . <br /> This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> t <br /> Describe a ions taken or will betaken to orrect violation: v> <br /> 0A k4APt�5y/j <br /> RECEIVED <br /> NOV , 2 2 2021 <br /> Page 3 of 4 ENVIRONMENTAL HEALTH <br /> DEPARTMENT <br />
