CCR 26583 2659 Failed to submit a training program or program is not reasonable or <br />adequate for facility <br />9. HSC 25508(a)(1) Failed to annually review and certify that the business plan is <br />complete, accurate, and up to date <br />10. HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to <br />appropriate personnel <br />11. Failed to submit revisions to business plan within 30 days of substantial changes in <br />operations <br />All penalties for these violations will be calculated pursuant to the appropriate California law. <br />Some applicable penalties may accrue on a daily basis unless and until the conditions are <br />abated. <br />If no action is taken to abate the conditions at the Facility as described in the inspection report <br />then EHD will file an Administrative Enforcement Order and Notice of Defense (hereinafter <br />"Order") against you. <br />However, EHD is extending you the opportunity to advise of any other information that you <br />believe should be considered before the EHD files an Order against the you. <br />Further, EHD is willing to discuss with you options for settlement of these issues without <br />further enforcement action. Therefore a meeting has been scheduled as follows: <br />DATE: July 28, 2021 <br />TIME: 10:OOam <br />LOCATION: San Joaquin County Environmental Health Department <br />1868 E. Hazelton Ave. <br />Stockton, California 95205 <br />If you do not contact EHD and provide EHD with additional information that is sufficient to <br />overcome the findings in the inspection report or a settlement proposal by the date above then <br />EHD intends to file the Order including the appropriate penalties. <br />Thank you for your prompt attention to this matter.
