S0 JOA U 1 Environmental Health Department <br /> U i� I I AMENDED <br /> 10/22/2021 <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> PG&E:West Side Substation 20168 Wicklund Rd,Tracy August 12, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS)annually by the due date, January 15.The facility, inventory, and plans <br /> section of the Hazardous Material Business Plan (HMBP)was last submitted on 1/16/2020.The HMBP was <br /> submitted late for the year of 2020.The facility, inventory, and plans sections of the HMBP have not been submitted <br /> for the year of 2021. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This violation has been corrected. <br /> This is a minor violation. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended on <br /> 10/22/2021 and replaces the initial report on 8/12/2021 and the amended inspection report issued on 8/13/2021. <br /> The inspection report completed on 8-12-2021 said no violations were found at time of inspection. Further <br /> review has determined the Hazardous Material Business Plan (HMBP) has not been submitted for the year of <br /> 2021. This is why violations#9 has been added to the inspection report. Violation#4 has been issued because <br /> the primary contact phone number is not correct. More details are provided in the comment section for each <br /> violation. Violation#4- 8 were rescinded. Violation#9 has been changed from class 2 violations to <br /> minor violations. Violation#9 haas been corrected. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
