SHIPPING PAPER INSTRUCTIONS <br /> This shipping paper form is for the use of LEU employees in compliance with H&SC Sections 251 JO.10,25121.3 and 25163.3 <br /> 1. Enter location and date waste was picked up. <br /> 2. Enter quantity and type of container for each waste being transported. <br /> For example: 2/55 gal drums <br /> 3. Enter corresponding estimated weight or volume of waste being transported. <br /> For example: 110 gallons (liq uid) or 400 pounds solids <br /> Guide for Estimating Weights and Volumes of Material <br /> 55 gallon drum soil/debris 800 lbs <br /> 55 gallon drum liquid 450 lbs <br /> *The maximum hazardous waste allowable per vehicle is 2,500 pounds of solid or 500 gallons of liquid unless <br /> non-PCB mineral oil(5,000 gallons allowed) or liquid from vault de-watering,(1,600 gallons allowed). <br /> 4. For Non-PCB oily wastes, circle all descriptions that apply: <br /> For example: Non-PCB Oily Solids - dirt, sweep, rags, absorbent, debris, filters, rocks <br /> contaminated with transformer oil less than 5 ppm, hydraulic oil, motor oil, lube oil or turbine <br /> oil. <br /> 5. Date, print, and sign your name below the Transporter's Certification statement. <br /> 6. Deposit shipping paper in mailbox at the south east comer of the 90 Day Hazardous Waste <br /> Accumulation Disposal Area next to the transformer/meter shop and raise the red flag. <br /> 7. Receiving employee will date and sign shipping paper, keep original for records, and forward <br /> copy to the Environmental Compliance Officer. <br /> Note: If you have Non-RCRA hazardous waste not included in the examples, or any other hazardous waste question, <br /> contact Environmental at 526-7321 or 526-7448. <br /> LABELING REQUIREMENTS/INSTRUCTIONS <br /> L Used Oil Label- For liquid oil containing less than 5 ppm PCBs and will be reused. <br /> Complete label and place on side of drum. (A hazardous waste label should not be applied to a <br /> drum for used oil when oil will be reused.) <br /> 2 Hazardous Waste Label - (Drums/containers only - Do not place on equipment) <br /> a. In addition to the preprinted generator information, enter the following information on <br /> the label: <br /> • Contents Composition: Write common name in bold frorn shipping paper <br /> • Physical State: Check box for solid or liquid <br /> • Hazardous Properties: Check boxes that apply (Please note: oil waste, oily waste <br /> and asbestos are "Toxic") <br /> b. Place the label on the side of the hazardous waste drum/container <br />
