SAN J OA U I Environmental Health Department <br /> COUNTY <br /> — <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ENVIROKEM ENGINEERING SERVICES, INC 4670 E WATERLOO RD, STOCKTON July 26, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: Two 55 gallon drums ofAmidex CE Surfactant, two 55 gallon drums ofAquamet M (sodium <br /> diemethyldithiocarbamate), One 265 gallon tote Neutrala CSLS 45%, four 55 gallon drums of AOS–40 (aquatic <br /> toxicity), two 55 gallon drums of glycolic acid 70%, two 55 gallon drums of glacial acetic acid, one 265 gallon tote of <br /> ferric chloride solution, one 55 gallon drum of degreaser, nineteen 50 pound bags of ferrous sulfate, two 55 gallon <br /> drums of ECO1200 (polyaluminum Hydroxchlorosulfate blend), one 55 gallon drum of potassium hydroxide, one 55 <br /> gallon drum of EC09000, and two hundred nineteen 50 pound bags of hydrated lime were found on site that have <br /> not been reported to CERS. <br /> REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> This is a Class II violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: A site map was not completed and submitted as part of the business plan. <br /> REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as "NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> OBSERVATION: The emergency response(ER) procedures have not been submitted. <br /> REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br /> release or threatened release of hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> FA0026720 PR0547112 SCO01 07/26/2021 <br /> EHD 19-01 Rev.9/16/2020 Page 3 of 5 Hazardous Materials Program OIR <br /> 1868 E.
