<br />13. Except as specifically provided otherwise by this license, the licensee shall possess and use radioactive material
<br />described in Items 6, 7, 8 and 9 of this license in accordance with statements, representations, and procedures
<br />contained in the documents listed below. The Department's regulations shall govern unless the statements,
<br />representations, and procedures in the licensee's application and correspondence are more restrictive than the
<br />regulations.
<br />(a) The application with attaclunents .received December 10, 1987, signed by W. Robert Sawyer, NLD.
<br />(b) The letters with attachments dated April 25, 1989, May 22, 1989, May 25, 1989, June 27, 1989, and
<br />September 6, 1989, all signed by Nathan I -L Woodruff, Jr., NLD.
<br />(c) The letter dated August 15, 1989, signed by Nathan H Woodruff, Jr., NLD.
<br />(d) The letter with attached Group 6 procedures dated May 17, 1990, signed by Anil Sain, NLD.
<br />(e) The letter dated April 28, 1992, signed by Anil Sain, NLD., regarding the change from quarterly to semi-
<br />annual consultants audits.
<br />(f) The letter with diagram dated May 13, 1992, signed by Anil Sain, NLD., regarding the addition of camera
<br />room No. 3.
<br />(g) The letter with attachments, dated December 17, 1993, signed by Anil K. Sain, NLD., regarding new
<br />camera.
<br />(h) The letter with attachment dated July 19, 1995 signed by Anil K. Sain, NLD., Radiation Safety Officer,
<br />regarding the nuclear medicine room floorplan and purchase of a new gamma camera.
<br />14. (a) The Radiation Safety Officer in this program shall be Anil K. Sain, NLD.
<br />(b) The Chairperson of the Radiation Safety Committee shall be Anil K. Sam, NLD.
<br />(c) The Custodian of sealed sources shall be Edward C. Greenleaf, NLD.
<br />(d) The Alternate Radiation Safety Officer in this program shall be Chull Song, NLD.
<br />15. Sealed sources possessed under this license shall be tested for leakage and/or contamination as required by
<br />Title 17, California Radiation Control Regulations, Section 30275 (c).
<br />16. Quantitative analytical assays for the purpose of tests for leakage and/or contamination of sealed sources shall
<br />be performed only by persons specifically authorized to perform that service.
